Terms of Service

Agreement
These Terms of Service (“Terms”) are a contract between you and BOIntake (“we”, “us”) for use of the BOIntake platform. If you do not agree, do not use the Service.
BOIntake is the trading name of the Service at bointake.com. Contact us at support@bointake.com or +959 4000 833 16.
1. The Service
BOIntake is a booking and order intake platform. Enterprises can:
- Register a workspace and start a 14-day free trial (starter limits apply)
- Design public intake forms (name is always required; phone and other fields may be required, optional, hidden, or omitted)
- Share a public URL so guests can submit without an account, or mark a designed form as invite-only and share unique guest links (one pending request per invite; reuse only after cancel)
- Manage submissions, update status, and (when tracking is enabled on the form) let guests track a request with a reference code and track token, or edit a pending request with an edit key
- Invite sub-admins within plan limits
- Request paid plans or wallet credits through in-app billing
The Service is offered in English and Burmese (my-MM). Pricing may be shown in MMK or USD.
2. Eligibility and accounts
You must be able to form a binding contract. If you register for an organization, you confirm you have authority to bind it.
Workspace users sign in with email or username and password, or with Google where that option is enabled. Self-registered accounts and enterprise-created admin accounts must verify email before using the workspace. Accounts created by a platform super-admin may skip that step. Changing a workspace user’s email requires verifying the new address before they can use the workspace again.
You are responsible for keeping credentials confidential, activity under your account, choosing a password of at least 8 characters, and telling us promptly if you suspect unauthorized access. We may suspend or deactivate accounts that are inactive, unpaid, abusive, or in breach of these Terms.
3. Workspaces and guest submissions
Each enterprise is a separate workspace. Admins, forms, and submissions for one enterprise are not shared with other enterprises.
You (the enterprise) are responsible for what your public forms ask guests, how you use guest data (including extra fields you add), notices or consents required for your guests, accuracy of statuses you set, and keeping track tokens, edit keys, and invite links reasonably confidential. Anyone with the reference code, track token (or track link), and your public track page can view that request. Treat the reference code like a username and the track token like a password. Anyone who also has the edit key can change a pending request. You can turn guest tracking off on a designed form so guests are not issued tracking codes. Invite-only forms are not shown or accepted without a valid invite token. The same invite can be used again only after cancel; a confirmed or completed request on that invite makes the link invalid.
Guests have no BOIntake account. Their relationship for the booking or order is with your organization, not with us, except as needed for us to host the form.
4. Plans, trial, credits, and billing
4.1 Trial and plans
New enterprises start on a free trial with included limits (admins, custom forms, and submissions). Paid plans (for example Growth and Enterprise) increase those allowances. After included usage, extra usage may consume wallet credits where the product allows it. We may change plan catalogs, limits, and prices with reasonable notice for the next billing period. Included limits usually apply from the next cycle unless a platform operator applies them to in-cycle tenants immediately.
4.2 How payment works
The current product does not charge cards automatically. To upgrade or buy credits you submit a billing request (plan or credits, region, currency, optional contact via Viber, Telegram, WhatsApp, or email, message, and payment screenshot).
A platform operator reviews the request and may approve or reject it. Access to paid features is granted after approval, not when you upload a screenshot. Amounts shown at submit time are a snapshot; we may correct obvious errors. You are responsible for paying any bank fees, foreign-exchange, or tax on your side.
4.3 No refunds
Unless required by law or we agree in writing, fees and credited wallet amounts are non-refundable after approval.
4.4 Expired trial or subscription
If the trial or paid period ends, included allowances may stop. Some actions may still be possible using remaining wallet credits, or the workspace may be limited until you upgrade or we reactivate it.
5. Acceptable use
You must not:
- Break applicable law or collect data you are not allowed to collect
- Upload malware, or try to probe, disrupt, or bypass the Service
- Impersonate others or misrepresent your organization
- Share login credentials in a way that violates these Terms
- Use the Service to send spam or abusive content
- Infringe others’ intellectual property or privacy
- Resell the Service except as we agree in writing
- Attempt to access another enterprise’s workspace
We may remove content, close public forms, or suspend workspaces that violate this section.
6. Your content and our software
Your content includes enterprise profile data, form designs, uploaded images and logos, guest submissions (including file attachments on paid plans), and billing uploads. You keep ownership of your content. You grant us a license to host, process, display, and back it up as needed to provide the Service.
Our software, branding, and documentation remain ours. You may not copy, reverse engineer, or resell them except as allowed by law. Public form URLs, logos, and form images may be reachable by anyone who has the link. Do not upload secrets onto public forms.
7. Privacy
Our Privacy Policy explains how we process personal data. By using the Service you also acknowledge that policy.
8. Availability and changes
We aim for a reliable Service but do not guarantee uninterrupted or error-free operation. We may change, suspend, or discontinue features. We may perform maintenance without notice when reasonably necessary.
Capacity limits, acceptance windows, and “fully booked” or “booking time over” states are tools you configure. We are not responsible if a form stays open or closed contrary to your operational needs because of misconfiguration.
9. Disclaimers
The Service is provided “as is” and “as available.” To the maximum extent permitted by law, we disclaim warranties of merchantability, fitness for a particular purpose, and non-infringement.
We do not warrant that guest submissions will always be received, stored, or delivered; that track lookup cannot be used by someone who obtained the reference code and track token; that billing screenshots or manual payments will be processed within a particular time; or that the Service is suitable for regulated sectors (for example healthcare) unless you add your own compliance measures.
BOIntake is an intake and operations tool. It is not a payment processor, clinic system of record, or legal filing service.
10. Limitation of liability
To the maximum extent permitted by law, we are not liable for indirect, incidental, special, consequential, or punitive damages, or lost profits, data, or goodwill. Our total liability for claims relating to the Service is limited to the fees you paid us for the Service in the 3 months before the claim (or USD 50 or equivalent if you are on a free trial and paid nothing).
These limits do not apply where the law does not allow them (for example death or personal injury caused by negligence, or fraud).
11. Indemnity
You will defend and indemnify BOIntake against claims arising from your content, your guests’ data, your use of the Service, your breach of these Terms, or your violation of law or third-party rights.
12. Termination
You may stop using the Service at any time. We may suspend or terminate access for breach, non-payment, legal risk, or if we shut down the product.
On termination we may disable public forms and admin access. We may delete or anonymize data after a reasonable period, except where we must retain it. Soft-deleted submissions are permanently removed after 30 days. Users and forms stay in the workspace until you request permanent erasure, or we delete them after termination.
13. Governing law
These Terms are governed by the laws of the Republic of the Union of Myanmar, without regard to conflict-of-law rules. Courts in Yangon have exclusive jurisdiction, except that we may seek injunctive relief anywhere to protect the Service.
14. Changes to these Terms
We may update these Terms. Continued use after the updated “Last updated” date means you accept the new Terms, unless applicable law requires a different process.
